The Change Well Project is excited to launch the Connection Hub, specifically designed for Housing Case Managers, Disability Benefit Advocates, and other direct service providers working in the CDSS-funded housing and services sector, which includes HDAP, HSP, BFH, and Home Safe.
The Connection Hub is a collaborative and supportive space where housing case managers and direct service providers can share knowledge and strategies to address the complex needs of their participants, particularly around housing and support services. By facilitating peer learning, consultation, and networking, the Connection Hub will provide support to help you navigate case management-related challenges and develop effective, participant-centered solutions.
During these sessions, you will have the opportunity to:
Present and Discuss: Share and get support for navigating complex participant or household situations.
Enhance Skills: Practice case management skills to support professional development.
Ask Questions: Address “burning” questions and receive guidance.
